Methylcobalamin: The Neurotropic B12

Methylcobalamin (MeCbl) is one of two active coenzyme forms of vitamin B12 (the other being adenosylcobalamin). It acts as a cofactor for methionine synthase, which converts homocysteine to methionine, essential for myelin synthesis. It also plays a role in DNA synthesis, fatty acid metabolism, and energy production.

Neuroprotective effects: Promotes axonal regeneration, enhances Schwann cell proliferation, and improves nerve conduction velocity. Clinical studies show significant improvement in neuropathic symptoms (numbness, tingling, burning) after 4–8 weeks of treatment.

Homocysteine reduction: Elevated homocysteine is a risk factor for stroke, dementia, and neuropathy. Methylcobalamin lowers homocysteine levels, providing vascular protection.

Intramuscular injection bypasses gastrointestinal absorption issues (common in elderly, atrophic gastritis, Crohn's disease, post-gastric surgery), achieving 100% bioavailability.

Recommended Dosage (as directed by physician)

Diabetic neuropathy / peripheral neuropathy: 1500 mcg intramuscularly three times per week for 4 weeks, then 1500 mcg once weekly for maintenance.
Vitamin B12 deficiency (megaloblastic anemia): 1500 mcg intramuscularly once daily for 1 week, then 1500 mcg once weekly for 4 weeks, then 1500 mcg monthly.
Herniated disc / radiculopathy: 1500 mcg intramuscularly daily for 10 days, then alternate days for 2 weeks.
Trigeminal neuralgia: 1500 mcg intramuscularly three times weekly for 8 weeks.

Administration Instructions

For IM injection: Inject deeply into the gluteal or deltoid muscle. Aspirate to avoid intravascular injection. For IV injection: May be given slowly (over 2-3 minutes) or added to IV infusion fluids. Protect the solution from light during infusion. Use immediately after opening; discard any unused portion.

Methylcobalamin is extremely safe, even at high doses. Rare side effects may include:

Pain or redness at injection site
Mild diarrhea or nausea (very rare)
Allergic reaction (rash, urticaria – extremely rare)
Hypokalemia (with rapid correction of severe anemia)

⚠ Important Precautions

Hypokalemia: When treating severe megaloblastic anemia, vitamin B12 can cause rapid erythropoiesis, leading to hypokalemia. Monitor serum potassium levels during initial therapy.
Leber's optic atrophy: Methylcobalamin is generally contraindicated in patients with Leber's disease (hereditary optic nerve atrophy) as it may worsen symptoms.
Pregnancy & lactation: Safe in recommended doses (FDA Category C – no risk in humans). Consult physician.

⚠️ Contraindications: Known hypersensitivity to methylcobalamin or any excipient. Leber's optic atrophy (use with extreme caution). Not for intrathecal administration.
